Community Care Access Centres (CCACs)
Community-based health care is available for people who need health-care assistance at home. Community Care Access Centres (CCACs) co-ordinate services for individuals in their own community by helping people to live independently at home, apply for medical services or to a long-term care home, or by providing information about community support services agencies in their local communities. Some of the home-care services provided by the Community Care Access Centre (CCAC) include assistance with bathing and dressing, or professional services from a physiotherapist, nurse, or social worker.
